Depends upon the movie and who is doing the commentary. John Carpenter's commentaries are always gold, especially if he's doing the commentary with Kurt Russell. Also, the commentary done by Jim & Marion Lovell on APOLLO 13 is fantastic.
Depends on the movie/TV show, and who's doing the commentary. If I really like the TV show I'll generally listen to commentary on a couple episodes, particularly if the cast is doing commentary.
For movies it has to be the cast to make me listen. There's nothing more irritating than 2 hours of George Lucas saying, "look how awesome this movie looks. I'm brilliant"

I echo what the others have said. Some commentaries are really interesting, but some are no good at all. A lot of it depends on the show.
